Position Summary

  • Enhance the overall patient & family experience by coordinating and facilitating the HFC patient care team processes to meet the care needs and achieve quality clinical outcomes for all patients.

Key Qualifications

  • Registered in good standing with the College of Nurses of Ontario (CNO) or currently meet the eligibility criteria to register in good standing in the Temporary Class or General Class with the CNO
  • BScN Preferred
  • Minimum of 5 years of sound clinical experience with 3 years cardiac experience required
  • Previous formal leadership experience preferred
  • ACLS preferred
  • Extensive knowledge of cardiac disease process
  • Knowledge of heart failure pathway and treatments
  • Strong communication skills along with the ability to establish positive relationships/partnerships with all staff, patients, families, internal and external partners
  • Demonstrates initiative, creativity and innovation in clinical practice
  • Excellent critical thinking and decision making skills
  • A demonstrated commitment to patient centered care
  • Ability to promote change and support and mentor others through the change process
  • Demonstrates continuous learning and professional development
  • Exemplary performance record

Position Responsibilities

  • Your leadership, facilitation, decision making, conflict resolution and clinical abilities will promote and encourage staff empowerment
  • Plans, organizes and co-ordinates the day to day activities within the Heart Function Clinic
  • Provides clinical support to staff and patients
  • Identifies gaps in service and makes recommendations to address issues
  • Consults and facilitates with community agencies if required
  • Monitors quality indicators including patient satisfaction
