  • Job Description: Reporting to the Manager/Director, First Nations Métis Health, the Clinical Outreach Nurse is an integral member of the team, providing and coordinating initial, continuing and comprehensive nursing services to patients within partnering First Nations' communities/Tribal Councils and Health Networks. This work is done in close collaboration with the clients, including First Nations individuals living on/off First Nation Communities, non Indigenous clients, urban/rural/remote communities. The Clinical Outreach Nurse promotes quality health care outcomes working within established standards for clinical nursing practice. The Clinical Outreach Nurse initiates and/or participates in community development initiatives respecting First Nation's principles of OCAP® (Ownership, Control, Access and Possession and is a Registered Trademark of the First Nations' Information Governance Centre (FNIGC).   • Graduate from an approved School of Nursing.
  • Baccalaureate Degree in Nursing.
  • A valid CPR Basic Life Support Level “C” certificate.
  • Experience
  • Minimum two (2) years of recent nursing experience in a community based setting is required. Previous experience in teaching, facilitation skills and leading group activities is required. Experience with and/or demonstrated knowledge of Indigenous cultural and healing traditions is required including a focus on primary healthcare and individuals who are at risk for and/or are living with chronic and/or communicable diseases. Experience in areas including, but not limited to primary care, prenatal care, chronic and communicable diseases, general infectious diseases and case management is an asset.
  • Knowledge, Skills and Abilities
  • Knowledge: Understands levels of prevention and how these are used in promotion, prevention and protection of health. Demonstrates knowledge of multiple health promotion strategies including building healthy public policy, creating supportive environments, strengthening community action, developing personal skills, and reorienting health services. Knowledge of the importance of culture on health. Knowledge of Indigenous culture and traditional healing practices specifically within First Nations' communities/Tribal Councils including but not limited to sexual health and public health (specifically STBBIs), acute care medicine, pre and post natal care, primary health care, chronic diseases care and general infectious diseases care. Knowledge of the interests and health needs and priorities of the local First Nations' communities/Tribal Councils. Knowledge of Cultural Responsiveness Framework and the Truth and Reconciliation Commission of Canada's Calls to Action. Demonstrates knowledge and /or ability in the following areas: community development, community assessment, educational resource development, media communications and social marketing. Uses knowledge of social determinants of health in interactions with clients at all levels. Knowledge of harm reduction principles and practices.
  • Skills: Excellent triage and assessment skills. Demonstrates application of the nursing process in dealing with individuals, groups, communities, First Nations' communities/Tribal Councils, or organizations. Makes independent nursing judgements based on evidence based practice. Uses critical analysis, reflective practice, assesses and utilizes evidence based practice. Collaborates with the client at all levels (individual, group, community) to assist clients in taking responsibility for their own health. Analyzes health issues, especially related to health equity, in the political and social climate along with individual / community readiness. Evaluates nursing interventions in terms of individual, group and community outcomes. Demonstrates skill in: negotiation and conflict resolution, group education and facilitation, individual counselling, motivational techniques and advocacy.
  • Abilities: Ability to deal tactfully and effectively with people in a non judgmental manner. Ability to be self directed and exercise independent decision making in a community based environment. Ability to work collaboratively and effectively with team members and the community at large. Demonstrates flexibility by adjusting workload to accommodate client needs in a positive manner. Ability to be adaptable and flexible to meet program demand. Ability to communicate effectively orally and in writing, adapting to client needs or professional interactions. High level of accuracy is required in all aspects of assigned responsibility such as health assessment, nursing diagnosis and intervention, immunization, and handling specialized equipment. Ability to demonstrate appropriate level of professional conduct and judgement.

  • Ability to maintain a current registration with the College of Registered Nurses of Saskatchewan (CRNS) prior to employment.
  • A valid Class 5 drivers’ license and have a vehicle available for work
