- Job Description: The Mental Health Therapist - Staff is an integral member of the Primary Health Care Team and works in part of the network team in the medical home model. As a member of a multidisciplinary primary health care team, the incumbent is primarily responsible for behavioral health supports for a number of conditions under the care of the Primary Health Care Provider. The services to be delivered through this position include screening functional assessment and develops behavioral change plans in conjunction with the Primary Health Care Provider. The Mental Health Therapist - Staff develops and implements a behavioral healthcare plan and makes referrals as required.

- Familiar with community resources and services
- Experience w/ clinical interventions in Integrated Care or Community Health Care
- Experience in brief treatment modalities in PHC setting is an asset
- Experience with clinical interventions related to Primary Health Care
- Education
- Bachelor of Science in Human Services (BSHS)
- Licenses
- Valid Class 5 driver's license

- Kindersley is located approximately 200 km west of Saskatoon
- Registered/eligible for registration with the appropriate licensing body
