Project Overview We are building a Maturity Assessment workflow that begins with a JotForm questionnaire and ends with an automatically generated Findings Report. We are looking for someone who is experienced, detail-oriented, and available to start immediately with fast turnaround times. Scope of Work You will be responsible for setting up an automated workflow that includes: JotForm Build-Out Convert ~60 assessment questions into a well-formatted JotForm with logic, weighting fields, and clean layout. Ensure the form is easy to use and produces structured output for integration. Data Handling & Spreadsheet Structure Store form results in a secure and protected Google Sheet or Excel equivalent. Apply formulas for weightings, scoring calculations, and maturity results (we will provide the scoring logic). Chart Generation Using QuickChart Take calculated results and automatically generate visual charts (bar charts, radar charts, etc.) using QuickChart.io API. Ensure charts render cleanly and can be inserted into the Findings Report. Findings Report Generation in DocuPilot Map all calculated values + charts into a DocuPilot template to create a polished client-ready PDF report. We will provide the template. Automation with Zapier Use Zapier to automate the workflow end-to-end: Trigger on form submission Push to spreadsheet Generate charts Populate DocuPilot template Output ready-to-download PDF Required Skills & Experience -Proven experience integrating Zapier workflows from start to finish -Strong JotForm form logic / calculation experience -Experience working with QuickChart or similar chart APIs -Experience mapping data into document automation platforms (e.g., DocuPilot, Formstack, or similar). We will provide the Findings Report template — you will be responsible for correctly mapping fields and chart images. -Strong attention to detail and ability to handle structured data reliably -Able to work independently and proactively, with minimal back-and-forth Nice to Have (Not Required) -Familiarity with business / consulting report formatting -Experience with maturity models, scoring rubrics, or structured assessments Deliverables -Fully functioning automated workflow -Clear documentation / handoff instructions -Ability to troubleshoot and refine over first live client runs Timeline Immediate start Preference for rapid turnaround (initial version within 5–7 days) How to Apply Please include: Examples of similar automation workflows you have built Specific platforms you have integrated (Zapier, JotForm, QuickChart, DocuPilot, Google Sheets, etc.) Your availability and expected timeline to complete this project Your hourly rate or project-based quote
