  • We are currently seeking dynamic and strategic Registered Nurses (RN) to join our team as an Integrated Care Program Supervisors (evening shift) to lead a diverse, multidisciplinary team that will deliver high-quality, integrated and palliative care in the Mississauga community and surrounding area. Working closely with health system partners, you will be an integral part of shaping the future of health care in Ontario. If you are a transformational leader, we want you on our team!
  • This role requires evening coverage: 4:00 pm - 12:00 am.
  • Job Description
  • Manage and coordinate integrated palliative care services within a designated neighborhood, ensuring continuity of care and optimal patient experience.
  • Collaborate with your team to assess, plan, and coordinate care plans that support patients' end-of-life wishes, including preferences for place of death and MAID requests.
  • Oversee both direct and indirect care coordination activities, supporting your team in providing high-quality care, including pain management, advanced clinical skills, and interdisciplinary collaboration.
  • Build and maintain relationships with patients, caregivers, and key stakeholders, ensuring seamless care approaches and addressing any questions or concerns.
  • Monitor and address quality, safety, and risk issues, developing improvement plans and participating in Quality-of-care Reviews as required.
  • Support program development, implementation, and evaluation to enhance care quality and team performance.
  • Qualifications
  • Registered Nurse (RN) in good standing with the College of Nurses of Ontario.
  • Minimum of 5 years nursing experience in a community setting and 2+ years in a management role
  • Must have experience working with palliative care patients
  • Experience with program development, implementation and evaluation
  • Experience managing teams in a unionized environment an asset
  • Experience working with care coordination
  • Valid G driver's license and reliable vehicle
  • Must be available for on-call rotation after hours and weekends
