Position Summary:

The Intake Manager plays a critical role in managing all aspects of the client intake and admissions process for a Medicaid-certified home care agency. This role is responsible for ensuring compliance with Medicaid regulations while coordinating timely and efficient onboarding of new clients. The ideal candidate brings experience with Medicaid-funded home care services, strong organizational skills, and a compassionate, client-centered approach.

Key Responsibilities:

  • Serve as the lead for all new Medicaid client referrals and intake inquiries.
  • Oversee the end-to-end intake process, ensuring all eligibility, documentation, and compliance requirements are met.
  • Verify Medicaid eligibility and work closely with case managers, service coordinators, managed care organizations (MCOs), and state agencies to authorize services.
  • Schedule and coordinate in-home assessments and ensure the completion of all required documentation.
  • Ensure accurate data entry and client tracking in the EMR or Medicaid billing system (e.g., HHAeXchange, eMedNY, AlayaCare).
  • Maintain communication with referral sources such as hospitals, SNFs, social workers, and managed care providers.
  • Collaborate with scheduling, nursing, and care teams to support timely service starts and optimal care coordination.
  • Monitor and track pending intakes, authorization statuses, and service start timelines.
  • Ensure adherence to state Medicaid guidelines, HIPAA, and agency policies at all stages of intake.
  • Provide ongoing training, support, and supervision to intake coordinators or assistants.
  • Assist in auditing charts and documentation to ensure compliance and readiness for Medicaid reviews or audits.

Qualifications:

  • High School Diploma or GED required; Associate’s or Bachelor's degree in Healthcare Administration, Nursing, or a related field preferred.
  • Minimum of 2–3 years’ experience in a home care intake, admissions, or case management role—Medicaid home care experience required.
  • Strong understanding of Medicaid-funded programs (including CDPAP, PCA, HHA, and MLTC).
  • Familiarity with EVV systems, MCO portals, and prior authorization workflows.
  • Excellent interpersonal, customer service, and communication skills.
  • Detail-oriented with strong organizational and time management abilities.
  • Proficiency in home care software platforms such as HHAeXchange, AlayaCare, or ClearCare is strongly preferred.
  • Bilingual skills are a plus (especially Spanish, Haitian Creole, Russian, or Mandarin).

Pay: $60,000.00 - $95,000.00 per year
