- Qualicare HomeCare BC is seeking a compassionate and organized Registered Nurse (RN) or Licensed Practical Nurse (LPN) to support our clients through comprehensive intake assessments and monthly follow-up visits. This role is ideal for an RN who enjoys building relationships, coordinating care, and using clinical judgment to guide care plans in a home care setting.

- About Qualicare HomeCare BC

- Your Role
- As an RN in this role, you will focus on clinical assessments, care coordination, and monitoring client progress, rather than routine bedside nursing tasks.
Key Responsibilities
- Conduct initial client intake assessments in the home or community
- Complete monthly follow-up visits to reassess client needs and progress
- Develop, review, and update individualized care plans
- Collaborate with Care Coordinators and caregiving staff to ensure care aligns with clinical recommendations
- Provide clinical guidance and support to the care team
- Maintain accurate, detailed, and timely documentation
- Identify changes in client condition and recommend appropriate interventions
- Ensure compliance with infection control standards and professional practice guidelines
- What We’re Looking For
- Licensed Registered Nurse in British Columbia (in good standing)
- Current CPR certification
- Strong assessment and care planning skills
- Excellent communication and interpersonal abilities
- Comfortable working independently in the community
- Strong organizational and documentation skills
- Negative TB test or chest X-ray
- Valid driver’s license and access to a vehicle
- This is a great role for an RN/LPN who prefers a consultative, assessment-focused position with flexibility and meaningful client interaction—without the physical demands of shift-based bedside care.
