  • Your Role
  • As an RN in this role, you will focus on clinical assessments, care coordination, and monitoring client progress, rather than routine bedside nursing tasks.

Key Responsibilities

  • Conduct initial client intake assessments in the home or community
  • Complete monthly follow-up visits to reassess client needs and progress
  • Develop, review, and update individualized care plans
  • Collaborate with Care Coordinators and caregiving staff to ensure care aligns with clinical recommendations
  • Provide clinical guidance and support to the care team
  • Maintain accurate, detailed, and timely documentation
  • Identify changes in client condition and recommend appropriate interventions
  • Ensure compliance with infection control standards and professional practice guidelines
  • What We’re Looking For
  • Licensed Registered Nurse in British Columbia (in good standing)
  • Current CPR certification
  • Strong assessment and care planning skills
  • Excellent communication and interpersonal abilities
  • Comfortable working independently in the community
  • Strong organizational and documentation skills
  • Negative TB test or chest X-ray
  • Valid driver’s license and access to a vehicle
  • This is a great role for an RN/LPN who prefers a consultative, assessment-focused position with flexibility and meaningful client interaction—without the physical demands of shift-based bedside care.
