- Job Description: Under the direction of the Manager of Chronic Kidney Disease and Home Based Therapies, the Dietitian will provide medical nutrition therapy which includes nutritional assessment, screening, developing evidence-based care plans, counselling, nutrition support and other needed nutritional interventions for patients and families living with kidney disease and other chronic health conditions. The Dietitian works collaboratively in an interdisciplinary out-patient and community based setting. The Dietitian works with both the peritoneal dialysis and home hemodialysis programs to create a patient and family centred environment and provision of quality care. "Preference for this position will be given to HSAS members in accordance with the terms and conditions of the SAHO/HSAS Collective Agreement"
