In this position, you will engage in collaborative work within the community, with: HAMSMaRT, Refuge Newcomer Health, Shelter Health Network, Canadian Mental Health Association and Good Shepherd, Hamilton as part of the Primary Care Access and Attachment Team (PCAAT).
