Qualifications

  • Education: A minimum of an Associate's Degree (Bachelors preferred) in a Human

Service field or full certification as an Addictions Counselor Experience

  • Associate's Degree: Five (5) years; Bachelor's Degree: Three (3) years; Master's
  • Degree: Two (2) years Certified Addictions Counselor (CAC): Five (5) years
  •  
  • Hours: One late night 12p-8p per week, rotating weekends, and rotating holidays.
  •  

Your responsibilities in this role may include

  • 1. Providing case management services, discharge planning and care coordination
  • for individuals transitioning from an American Society of Addiction Medicine
  • (ASAM) Level Residential Substance Use Disorder Treatment (SUD) Program (3.7,
  • 3.5, 3.3, 3.1)
  • Handle a caseload of clients to aid them in achieving adjustments to specific
  • barriers and situations of a difficult of complex nature by connecting them with
  • the appropriate community resources Duties may include, but not be limited to
  • providing intake services to clients; preparing reports, treatment plans,
  • discharge/ continuing care plans, and keeping necessary records within client
  • database and electronic health records Coordinating the activities of one or
  • more groups of clients Generating referrals and aftercare plans Work is
  • performed in accordance with established regulations, and policies and
  • procedures; however, this individual is expected to exercise considerable
  • initiative and independent judgment in achieving objectives The work will be
  • reviewed by the Program Director, Program Supervisor or Clinical Supervisor
  • through the completion of assignments.
