Qualifications
- Education: A minimum of an Associate's Degree (Bachelors preferred) in a Human
Service field or full certification as an Addictions Counselor Experience
- Associate's Degree: Five (5) years; Bachelor's Degree: Three (3) years; Master's
- Degree: Two (2) years Certified Addictions Counselor (CAC): Five (5) years
- Hours: One late night 12p-8p per week, rotating weekends, and rotating holidays.
Your responsibilities in this role may include
- 1. Providing case management services, discharge planning and care coordination
- for individuals transitioning from an American Society of Addiction Medicine
- (ASAM) Level Residential Substance Use Disorder Treatment (SUD) Program (3.7,
- 3.5, 3.3, 3.1)
- Handle a caseload of clients to aid them in achieving adjustments to specific
- barriers and situations of a difficult of complex nature by connecting them with
- the appropriate community resources Duties may include, but not be limited to
- providing intake services to clients; preparing reports, treatment plans,
- discharge/ continuing care plans, and keeping necessary records within client
- database and electronic health records Coordinating the activities of one or
- more groups of clients Generating referrals and aftercare plans Work is
- performed in accordance with established regulations, and policies and
- procedures; however, this individual is expected to exercise considerable
- initiative and independent judgment in achieving objectives The work will be
- reviewed by the Program Director, Program Supervisor or Clinical Supervisor
- through the completion of assignments.
